School Nurse

The Methacton School District Health Program facilitates the healthy growth and development of our district children.  This program prevents and detects health problems, and maintains and improves the health status of our students.

ATTENTION 11TH GRADE PARENTS:
  • All 11th grade public school students are required to provide an 11th grade physical at some point during their 11th grade school year.  This obligation can be fulfilled with a sports physical, print out from the office visit or the school physical form found in the Student Health Forms.   
  • Please provide an updated immunization record.

 

ATTENTION 12TH GRADE PARENTS:
  • All 12th grade public school students must provide documentation of a Meningococcal A,C,Y, W booster vaccine (Meningitis).  This vaccine is typically provided at the student’s 16-year-old well visit.  This documentation MUST be received prior to the student entering 12th grade.

 All documents can be sent via submitted by taking a picture of the documentation and email to the nurses, student drop off in Nurse’s office or US mail.

 

To help us provide the best possible care, please let us know when there is:

  • Extended illness
  • Hospitalization
  • Surgical procedure
  • Medication change
